What to take for migraine frequency
Graded by the strength of the evidence — strongest first. Nothing here is sold to you.
CoQ10grade B— Meta-analysis of RCTs: CoQ10 supplementation reduced migraine attack frequency (and, in some trials, duration) versus placebo; effect on migraine severity was less consistent. Trials are relatively small, so certainty is moderate rather than high.
Vitamin Dgrade B— Meta-analysis of 5 RCTs: vitamin D treatment significantly reduced headache days, attack frequency, headache severity, and migraine-related disability versus control in migraine patients. One of the more consistent positive signals for a vitamin D outcome in this dataset, though the evidence base is still just 5 trials.
Magnesiumgrade C— 10 oral-magnesium RCTs (n=789): reduced migraine frequency and intensity vs. placebo (odds ratios 0.20 and 0.27, no CIs given); some trials had inadequate randomization per the review authors. A separate IV arm covered acute attacks, not prevention.
Melatoningrade C— Systematic review: melatonin showed promise for migraine prophylaxis, reducing attack frequency in several trials, with a tolerability profile comparable to or better than some standard preventives. The evidence base is small and heterogeneous.
